Abstract
The purpose of this ATM is to (1) review the Lunar Seismic Profiling Experiment Safe-Arm Slide failure which occurred at the United States Naval Ordnance Laboratory (NOL). (2) evaluate the cause of the failure, (3) review the changes in Qual/Flight Slide design incorporated to eliminate problems, and (4) review the measures taken to ensure that the Qual/Flight slides were properly made.
